Waves 1–2: Questionnaire Sources and Permissions
Ten to Men’s Questionnaire Sources and Permissions is a database of measures and sources used in the participant questionnaires in Waves 1 and 2.

Wave 1 questionnaires
Four questionnaires were developed for the Wave 1 data collection:
- Adult Males (18-55 years)
- Young Men (15 to 17 years)
- Boys (10-14 years)
- Parents of the Boys (10-14 years). This questionnaire was separated into two parts:
- Part A – information about the study boy, and
- Part B – information about the household and parents.
Excluding the Boys (10-14 years), all were self-completed questionnaires. A computer assisted personal interview (CAPI) was used to collect the data for the Boys (10-14 years).

Wave 2 questionnaires
The same method as Wave 1 was used for the Wave 2 data collection, except that the questionnaire for the Parents of the Boys was not separated into Part A and Part B.

Wave 3 questionnaire
For Wave 3, only one questionnaire was used as most of the respondents were over 18 years of age. This wave was conducted online.
